Fastener & Anchor Pull-Test Report Templates
A ready-to-use Excel workbook for anchor and fastener pull tests: enter the job data and one row per fastener, and the sheet marks each test PASS or FAIL and builds the summary for you. Plus a printable field checklist and an index of the standards that govern the work.
What's in the download
| File | What it does |
|---|---|
pull-test-report-template.xlsx | Proof Test sheet: job header (project, site, client, tester, witness, equipment and serial, calibration due date, base material, anchor type, embedment, specification), required proof load, hold time and displacement limit, then 40 test rows. Each row is marked PASS when the applied load, hold time, displacement and failure mode all meet the limits, FAIL otherwise. Summary: count, pass/fail, pass rate, minimum and mean load, maximum displacement, overall result, signature lines. |
| Ultimate Load sheet for tests taken to failure: n, mean, sample standard deviation, coefficient of variation, min, max, characteristic value (mean − k × s, with k taken from your standard) and min/design load ratio. | |
| Calibration Log: one row per instrument; due date and OVERDUE / DUE SOON / OK status are calculated. Converter for kN, lbf, mm and in. Standards Index. | |
pull-test-report-template.pdf | Printable blank of the Proof Test sheet for the clipboard. |
pull-test-procedure-checklist.pdf | Before the job, at each fastener, after the job: calibration in date, reaction frame clear of the anchor's influence area, steady loading, failures reported to the engineer of record. |
standards-index.pdf | Which document covers what: ASTM E488, E1512, D1761, F606, E8, E4, D4541, C1583; ACI 355.2 and 355.4; ICC-ES AC193 and AC308; BS 8539; ISO 898-1, 7500-1 and 376. An index only, no text from any standard is reproduced. |
Yellow cells are inputs, white cells are formulas. Built with standard spreadsheet functions only (IF, COUNT, COUNTIF, AVERAGE, STDEV, MIN, MAX, TODAY), so it opens in Excel, LibreOffice Calc and Google Sheets; formulas checked in LibreOffice. Load units are picked once per sheet (kN or lbf, mm or in).
Who it is for
Field technicians, inspectors and QA staff who run proof or ultimate pull tests on post-installed anchors, fasteners in wood or masonry, and similar jobs, and who now write the report by hand or rebuild a spreadsheet for every job. The templates work with any pull tester.
The seller, Quality Tester Solutions LLC, is the company behind Force-Test, which makes fastener pull testers.
What the templates do not do
They record and summarise your results. Acceptance criteria, load levels, sample sizes and the k factor for characteristic values come from your specification, the anchor manufacturer and the governing standard; enter them in the yellow cells. Check the current edition of any standard with its publisher.
