Free record sheet from Quality Tester Solutions LLC (Force-Test). Excel version with automatic statistics, calibration log and standards index: calyx-ai.net/qa/

How a row is marked

A row is PASS when all four are true: the applied load is at least the required proof load, the hold time is at least the required hold time, the displacement is within the limit (if you set one), and the failure mode is blank or "None". Anything else is FAIL. A row with no load entered gets no result. This is the same rule the paid Excel workbook uses.

The sheet records results; it does not set them. The proof load, hold time, displacement limit and how many anchors to test come from your specification, the anchor manufacturer and the governing standard (for example ASTM E488/E488M, ICC-ES AC193 or BS 8539, whichever your job follows). Check the current edition with its publisher.

For tests taken to failure, use the ultimate-load statistics calculator (mean, standard deviation, CoV and mean − k × s). For what a finished report should contain, see the guide What an anchor and fastener pull-test report should include. Planning the job? The sample size and proof load calculator works out how many anchors to test and at what load from your spec, and the calibration due-date checker flags overdue testers. If your tester reads in psi or bar, the gauge pressure to load calculator gives the gauge target for the proof load.
